Your child's health history

safe at SMSA. 

Forms need to be

updated every school year, as your child grows,

and as their

medical needs change.

Nothing is more important to us than the safety of your children.  It is imperative that each child in the building is properly accounted for at all times. Running late or have an appointment?Contact our Main Office to let us know! 

SMSA provides technology resources to students to promote educational excellence through an integrated academic, spiritual, and extra-curricular program.  Each student

and guardian must sign

an agreement of

proper internet and Chromebook usage.

Student Transportation forms vary for each district, but must be filled out each year, usually by April 1st of the previous school year.  Please visit the Transportation Forms page for more information.
